We’re hiring a Patient Care Coordinator – a dedicated and organized professional who ensures seamless coordination of patient care, supports clinical teams, and serves as a vital link between patients, families, and hospice leadership. This role is essential to maintaining quality care and compliance while fostering compassionate communication. You lead with empathy, organization, and attention to detail. You thrive in a mission-driven environment and are passionate about supporting patients and families while ensuring clinical operations run smoothly.

Requirements

  • Previous healthcare experience in an administrative setting required
  • Minimum of 1 year experience in a clinical healthcare setting (hospital, clinic, or home health agency)
  • Reliable transportation to meet attendance and punctuality standards (driver’s license not required unless specified)

Nice To Haves

  • Hospice experience preferred
  • Strong background in home health, healthcare, or hospice preferred

Responsibilities

  • Maintain clinical responsibility for your team and act as liaison between the Director of Patient Care Services (DPCS), clinical associates, patients, families, and the community
  • Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments
  • Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy
  • Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals
  • Ensure compliance with local, state, and federal regulations and hospice standards
  • Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences
