About The Position

Unity Health Care is hiring a Patient Care Coordinator to support Internal & Family Medicine patients with complex medical and social needs. This role is essential to ensuring patient-centered, coordinated care for vulnerable populations. Working as part of an inter-professional care management team, the Patient Care Coordinator conducts outreach, care navigation, and follow-up services across clinical, hospital, home, and community settings throughout Washington, DC.

Requirements

  • High school diploma or GED required (college coursework in health or business preferred).
  • Minimum of 2 years of experience in care coordination, patient navigation, or a related role.
  • Experience working in hospital, outpatient, or community health settings preferred.
  • Strong interpersonal, organizational, and communication skills.
  • Knowledge of medical terminology and electronic medical records (EMR).
  • Comfortable working with diverse and vulnerable populations.
  • Able to travel to multiple clinical and community locations.

Responsibilities

  • Conduct patient outreach using strength-based, patient-centered motivational interviewing techniques.
  • Coordinate care for high-risk and medically complex patients, including post-hospital and emergency department follow-up.
  • Assist patients with scheduling appointments and navigating the healthcare system.
  • Identify and help remove barriers to care such as insurance, transportation, and social service needs.
  • Serve as a liaison between patients, primary care providers, specialists, hospitals, and community partners.
  • Manage a panel of complex patients to close gaps in preventive and chronic care.
  • Participate in interdisciplinary team meetings and case conferences.
  • Provide culturally appropriate health education and support.
  • Ensure accurate documentation and compliance with HIPAA requirements.
