About The Position

We are seeking a detail-oriented and organized Patient Coordinator to coordinate and manage the scheduling of infusion therapy services for our patients. The ideal candidate will possess excellent communication skills, a strong understanding of infusion therapy, and the ability to work collaboratively with healthcare teams to ensure timely and effective patient care.

Requirements

High school diploma or equivalent required
Prior experience in infusion therapy or clinical scheduling preferred.
Familiarity with electronic health record (EHR) system, preferably Alayacare
Strong organizational and time-management skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.

Responsibilities

Answer phones
Receive and manage faxes and emails
Help facilitate communication of supply lists between RNs and pharmacies
Manage and coordinate the scheduling of infusion therapy appointments for patients.
Collaborate with nursing staff and other healthcare providers to optimize appointment availability.
Help facilitate deliveries by the pharmacies to the patients
Communicate with patients to confirm appointments, confirm receipt of supplies and drug, and address any non-clinical questions or concerns.
Ensure patients are aware of their treatment schedules and any necessary follow-up.
Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
Document scheduling changes and communicate updates to relevant team members.
Maintain organized and complete patient charts, updated orders and document communication regarding patient care plans
Work closely with infusion nurses and medical staff to ensure appropriate staffing levels for scheduled procedures.
Participate in team meetings to discuss scheduling challenges and develop solutions.
