Patient Care Coordinator, Weekends
About The Position
Become a part of our caring community and help us put health first As a Weekend Patient Care Coordinator, you will: Develop/maintain contact with hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on CenterWell's services available in the market. Collaborate with program staff and other field sales resources. Participate in monthly program business development meetings, bringing relevant data, reports, and information about changes within accounts and referral sources. Assist program in timely processing of physician orders. Report to a Market Executive of Sales Work an 8 hour shift every Saturday and Sunday Use your skills to make an impact
Requirements
- RN/LPN/LVN license is required.
- Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
- Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.
- Work an 8 hour shift every Saturday and Sunday
Nice To Haves
- Bachelor's of Science in Nursing is preferred.
- Three years clinical experience is preferred.
