Be Heard: Share your ideas and help shape the way we work through improvement huddles, employee surveys, and town hall meetings Patient Care Coordinator – Pediatrics/ Women’s/Children's Services Essential Duties: Patient Care Coordination Assists nurses with patient-related questions and clinical support needs Facilitates patient flow for the department as it relates to admissions, discharges, visits, etc. Manages own patient assignment, if applicable (varies by location) Resource and Equipment Management Confirms availability of necessary patient equipment, supplies and / or medications Optimizes utilization of department and hospital resources Staff and Resource Management Creates and evaluates staffing plans and schedules Oversees smooth transitions between care providers and between shifts Ensures adherence to workplace protocols and procedures Quality Improvement and Professional Development Participates in quality improvement initiatives, including but not limited to data collection, planning, implementation, and outcome evaluation. May support investigation and resolution of specific outcomes, situations, or clinical care concerns. Fosters a positive professional practice environment. Mentors and coaches staff to attain optimal performance and provides input to Annual Discussion. May serve as a competency validator in general nursing and specialty practice setting

Requirements

  • Current license as a Registered Nurse in the State of Michigan.
  • Minimum of 2 years of relevant experience, 2 years in the specialty area preferred.
  • Actively pursuing a Baccalaureate degree in Nursing required for those hired into the role prior to October 1, 2018.
  • A BSN is required for those hired into the role after October 1, 2018.
  • Transition Plan for those currently in the role for meeting Educational Requirements: - Begin BSN within two years of October 1, 2011 - Complete BSN degree by October 1, 2018

  • Charge Nurse experience preferred.
