The Patient Care Coordinator plays a crucial role in ensuring high-quality healthcare delivery to patients. This position is designed to help patients understand every aspect of their care, ensuring they receive the necessary treatment while comprehending their medical conditions, medications, and other instructions. The coordinator will be responsible for coordinating patient-care services and may assist patients in deciding between treatment options and preparing for those treatments. In this role, the Patient Care Coordinator will develop and coordinate patient care programs, manage patient care, and assist the care team in setting goals for quality assurance and best practices. They will assess and screen patients, providing them with education about medical processes and procedures. Additionally, the coordinator will provide referrals for continuing care and help locate community resources for patients. With input from patients, families, and the care team, the coordinator can create outcome goals and an overall care plan. They may also assist in locating funding for special procedures or other patient needs. The ideal candidate will possess compassion, respect, teamwork, critical thinking skills, resourcefulness, leadership skills, and emotional stability. Other duties may be assigned as necessary.
