About The Position

The Patient Care Coordinator will support daily operations of a Hospice Care in the home care and facility setting. This includes daily administrative functions, ensuring great patient and family experience, managing CNA schedules and operations for providers, managing the plan of care needs for the CNA and communicating changes to the patients/families so all are informed on a timely basis of any changes.

Requirements

• Must have 2 years of practice at comparable level in a healthcare setting.
• Experience in elder health is preferred.
• High energy and strong work ethic required.
• Attention to detail is essential.
• Enjoy working with an interdisciplinary team.

Nice To Haves

• Associate’s Degree is desired.

Responsibilities

• Support daily operations of Hospice Care in home care and facility settings.
• Perform daily administrative functions to ensure great patient and family experiences.
• Manage CNA schedules and operations for providers.
• Work with the clinical team and executive leadership to ensure high quality of daily operations, supplies, and equipment.
• Provide overall support for the office team.
