About The Position
New York Medical and Diagnostic Center is seeking a dedicated and detail-oriented Patient Care Coordinator (PCC) to join our team. In this pivotal role, you will serve as the primary point of contact for patients, ensuring they receive the highest level of care and service throughout their treatment journey. Your responsibilities will include working closely with healthcare providers, patients, and their families to coordinate care, manage schedules, and facilitate effective communication among all parties involved in the patient's care. As a Patient Care Coordinator, you will be responsible for scheduling patient appointments and managing the coordination of follow-up care. You will provide patients with essential information regarding their treatment plans, procedures, and available healthcare resources. Additionally, you will assist patients in understanding their insurance benefits and obtaining necessary authorizations. It will be your duty to track and manage patient care plans, ensuring timely delivery of services and appropriate follow-up. Maintaining and updating patient records in compliance with HIPAA regulations is crucial in this role. You will also coordinate with external providers, specialists, and laboratories to ensure comprehensive patient care. Addressing patient concerns and resolving any issues related to their care promptly and professionally will be part of your daily tasks. Furthermore, you will educate patients on the importance of compliance with treatment plans and encourage their active participation in their healthcare. Monitoring patient progress and facilitating communication between patients and healthcare teams will be essential to ensure a seamless care experience.
