Get to Know Your Team: Hillcrest Hospital Pryor is a 48-bed acute care hospital that provides a range of health care services to the community, including emergency services, urology, ENT, gastroenterology, cardiology, orthopedics, general surgery and wound care.

Requirements

Graduate of an accredited technical training program.
Possession of a valid state Licensed Practical Nurse license.
BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.

Nice To Haves

A minimum of two years of professional nursing experience in a clinic setting.

Responsibilities

The care coordination process includes outreach to patients, generally outside of face-to-face office visits, to help them understand and manage their health, navigate the health system, and meet their health goals.
This position requires expertise in the practical nursing process including the ability to provide focused assessment of acute and chronic conditions, disease management education, and coordination of care across the healthcare continuum.
