Responsibilities

• Coordinate patient follow-up and care transitions across departments and community partners.
• Assist in data collection, tracking, and integration through electronic health record (EHR) systems.
• Support patient engagement initiatives that promote health literacy and equitable access to care.
• Participate in program evaluation, quality improvement, and health compliance projects.
