The HEAL Program is a new NYC H+H initiative to transition into being the primary provider of medical services to young people in city custody. NYC H+H employed providers will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC H+H system.

Requirements

  • High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization
  • Successful completion of training courses including the performance of phlebotomy and electrocardiography (EKG), or completion of a training program or programs that include the performance of phlebotomy and EKG, such as Phlebotomy Technician and EKG Technician training programs, a Patient Care training program, or a Medical Assistant training program
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
  • New York State Nurse's Aide certification (assignment to a long-term care facility)

Responsibilities

  • Implement Patient Care protocols including: vital signs, specimen collection, PPD placement and/or reading, hearing and vision testing as directed
  • Follow and demonstrate strict infection control guidelines to ensure proper clinic environment
  • Assist clinicians/providers during patient evaluations as needed, including acting as chaperone
  • Perform phlebotomy
  • Administer PPD's as ordered and as needed
  • Obtain and process laboratory specimens, per protocols
  • Review chart prior to patient's departure for follow-up appointments, prescriptions and/or referrals
  • Communicate with scheduling departments to coordinate external health care appointments
  • Ensure that all patients have been assessed and have understanding of all instructions prior to leaving the clinic
  • Participate with clinical staff for in-service training, to ensure that nursing staff functions meet quality improvement standards
  • Attend supervision, conferences, and agency meetings as directed
  • Implement recommendations from the Quality Improvement Team and Infection Control Committee, as requested
  • Dispose of medical and hazardous waste, as per OSHA guidelines and clinic protocols
  • Maintains temperature log books, medication inventory logs of lab and PPD log, quality control logs including for and CLIA-waived labs
