About The Position

Community Health and Wellness Partners (CHWP) is an integrated primary care community health center that utilizes a team-based care model, sharing patientcare responsibilities among members of a team. The Patient Care Assistant is responsible for maintaining an excellent attitude in participating of the day to day clinical operations of the health center under the guidance and support of the Clinical Coordinator. Responsible for the collection of data, administration of medications and treatments, and documentation of care provided as qualified. Supports the coordination of all activities of team members engaged in clinical care for patients under the directions of the Clinical Team Lead, Clinical Coordinator and/or Providers. Responsible for training like staff in team-based care and supports team members to stay focused on the organization’s Mission, Vision and Values.

Requirements

  • Registered or Licensed Practical Nursing Degree; or
  • Medical Assistant Certification/Licensure; and/or
  • Three to Five years of experience nursing in an ambulatory health care setting, health center or integrated primary care practice.
  • Valid Ohio driver's license.
  • Active Registered or Licensed Practical Nursing License within the State of Ohio; or
  • Current or active Medical Assistant Certification.
  • Demonstrates expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned.
  • Demonstrates knowledge and success in effecting overall clinical operations.
  • Experience/Knowledge of accreditation processes and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment.
  • Ability to manage multiple responsibilities and emergency situations successfully
  • Proficiency in knowledge of office management, use of computer, software packages, and office machines.

Responsibilities

  • Meet or exceed all measurable standards of care, including overall clinical, quality, and productivity goals.
  • Ensure accuracy, efficiency, and appropriate confidentiality with patient charting, related recordkeeping and administrative functions as assigned are maintained, current and relevant at all times.
  • Promotes attainment of specified goals through health education to patients/families and health care personnel as delegated by the Clinical Coordinator/RN.
  • Promote and exemplify excellent customer service to ensure that all clients/patients are served with a high level of customer satisfaction and care.
  • Understand and ensure all practices and procedures related to the delivery of medical services and nursing practices are consistent with contracted protocols and procedures, regulating entities (HRSA, HIPAA, PCMH, etc.) professional standards, and that the team-based care model is followed with efficiency.
  • Develop and ensure ongoing effective, positive relationships within and among all team members, , patients and their families.
  • Achieve commitments from patients to adhere to an effective plan of care, and ensure all patient services are delivered in a caring and professional manner.
  • Collaborate to provide quality care with members of the team assigned. Ensure the success team based care and that relationships with all clinic operations staff are highly respected.

  • Ensure all actions, job performance, personal conduct and communications always represent the organization in a highly professional manner.

  • Volunteer to serve the community by offering five (5) hours annually to a community service program.
  • Participates as a member of the team-based care model by being a strong team builder, working to support performance goals related to patient care.
  • Provides direct nursing or medical assistant care to patients as assigned and qualified to perform with excellence.
  • Supports care plans to ensure maximum patient outcomes by working with team members and the patient.
  • Completes all patient charting, and ensure all patient records are current.
  • Participates in quality strategies to evaluation compliance with standards and identify opportunities to improve patient outcomes.
  • Attends and participates in all meetings, patient conferences, planning sessions related to quality assurance, patient care and other related topics within the health center.
  • Attends continuing medical education to maintain best practices and licensure or certification requirements.
  • Functions in the delivery of patient care.
  • Contributes to and supports all members of the healthcare team with day to day clinical activities.
  • Supports Team members in meeting practice standards of quality and efficiency.
  • Handles appropriate positive and negative feedback to motivate, encourage and develop team based care.
  • Recommends changes to clinical procedures and processes for the success of the Team care model and the organization overall.
  • Serves as a member on one of CHWP’s organizational committees.
  • Demonstrates a high level of skill at building relationships and customer service.
  • Demonstrates interpersonal savvy and influence skills in managing difficult patients.
  • Demonstrates high degree of knowledge and competency in integrated primary care.
  • Demonstrates a high level of problem-solving skills to better serve patients and staff.
  • Adheres to evidence-based practices such as patient safety, infection control and clinical quality.
  • Strong attention to detail and accuracy.
  • Excellent verbal and written communication skills.
  • Ability to implement and evaluate operational and administrative processes.
  • Participates in aspects related to quality compliance, improvement and risk management for a community health center.
  • Engages in certification projects, i.e. PCMH.
  • Engages in all compliance requirements of Community Health & Wellness Partners.
  • Any and all other duties as assigned by Leadership, Management or Administration.
