About The Position

We are seeking a dedicated Patient Assistant to join our healthcare team. The ideal candidate will have experience in a nursing home, home community care, or a teaching hospital setting. This position offers an opportunity to work directly in the patient’s home environment.

Requirements

• Experience in nursing home or home community care preferred
• Familiarity with basic medical documentation
• Willingness to receive on-the-job training
• Opportunity to earn your PSS certification — we provide training!
• Strong communication and interpersonal skills
• Basic phone and computer skills

Responsibilities

• Assist patients with activities of daily living and personal care like cooking a meal, light cleaning such as vacuuming and dishwashing, etc.
• Monitor and record vital signs
• Provide support in the patient's home and assist with errands, appointments, shopping, and light housekeeping
• Assist with medical documentation and record-keeping
• Collaborate with the patient, family, and members of our home health agency to ensure patient well-being
• Provide companionship
