Patient Care Assistant II
About The Position
As a Patient Care Assistant, you will help patients feel safer and more comfortable by assisting with their basic daily life activities. This includes directly supporting patients with tasks such as personal hygiene, mobility, and feeding, while also assisting nursing staff with vital sign monitoring and preparing patient rooms. Your success in this role comes from your genuine desire to help patients feel better, whether that means giving them fresh bed linens or checking their vitals. You're diligent about your tasks, observant of patients’ conditions improving or declining, and accept coaching well. The position is located at St. Luke’s Health - The Woodlands Hospital, a 245-bed facility serving North Harris and Montgomery counties. The hospital has been recognized as a Best Hospital by U.S. News & World Report, serves as the clinical home for the Dan L Duncan Cancer Center, and was awarded an “A” hospital safety grade by The Leapfrog Group in both 2023 and 2025. Clinical services provided include cardiovascular services, diagnostic imaging, women’s services (digital mammography and bone density studies), neurosciences, pediatric care (Newborn and Level III nurseries), surgery, pathology and pulmonary services, and sleep disorder services.
Requirements
- High School Graduate General Studies, upon hire or High School GED General Studies, upon hire
- graduated from an accredited school of nursing or 1 year direct patient care experience , upon hire
- Basic Life Support - CPR, within 14 - days
Responsibilities
- Provides direct patient care as delegated by RN staff
- Monitors and records vital signs
- Assists in providing personal hygiene and physical care needs
- Accurately measures, records, and reports intake and output
- Obtains and records daily weight
- Performs blood glucose testing using blood glucose meter
