About The Position

Siloam Health’s mission is to share the love of Christ by serving those in need through health care. Our patients come from more than 80 countries and speak over 50 languages. Siloam’s approach to medical care provides a unique, interdisciplinary model of whole-person health care that addresses the physical, social, emotional, and spiritual determinants of health. Our staff is anchored by a multicultural primary care team that serves alongside roughly 300 volunteer interpreters, doctors, nurses, and other licensed medical specialists each year to provide comprehensive health care to Nashville’s most vulnerable. The Patient Assistance Coordinator serves Siloam Health’s patients in two areas: 1) the Patient Drug Assistance Program (PDAP) which provides free medication from pharmaceutical companies to eligible patients at Siloam Health Melrose (SHM), and 2) the coordination of referrals to off-site partners for specialist treatment not offered at Siloam Health Antioch (SHA).

Requirements

  • Bachelor's degree or minimum of 4 years of experience in an office setting required.
  • Passionate about Christian ministry to the underserved and are in full agreement with Siloam’s mission and core values.
  • Finds enjoyment in completing administrative tasks and helping others to succeed.
  • Ability to multi-task and prioritize projects daily.
  • Ability to follow directions, work with a team and demonstrate trustworthiness.
  • Adherence to OSHA and confidentiality requirements.
  • Computer literacy and familiarity with MS Office products as well as electronic medical records preferred.
  • Willingness to serve persons with limited access to healthcare and who are from different cultures and faith beliefs

Nice To Haves

  • Familiarity with medical terminology is preferred.

Responsibilities

  • Manage the Patient Drug Assistance Program for SHM patients which includes:
  • Monitor and process enrollment requests from providers via the EMR by
  • Verifying patient demographic information in the EMR is up to date
  • Confirming adequate proof of income is provided to meet the eligibility requirements for each pharmaceutical company
  • Follow-up on submitted applications, enrollment deadlines, re-enrollment applications, and refills
  • With the assistance of nurses and clinic volunteers, receive and process medications as they are delivered to Siloam
  • Maintain a spreadsheet on SharePoint of medications received which documents patient notifications (Weave text or phone call) and is updated with the date of pick-up and initials of staff giving the meds
  • Maintain an accurate PDAP database along with associated documentation
  • Maintain clear communication and transparency with SHM providers, the clinical team, and the pharmacy on PDAP cases via the EMR
  • Serve as the primary point of contact for patient PDAP questions
  • Create periodic summary reports
  • Meet with Melrose clinical staff to provide a PDAP refresher and answer pertinent questions at least twice a year.
  • Provide training for volunteers on the proper procedures for handling and storage of PDAP medications.
  • Other duties as assigned by supervisor.
  • Initiates referral process and evaluates requirements for each referral per protocol.
  • Ensures that referrals are addressed in a timely manner.
  • Provides appropriate clinical information to the specialist.
  • Documents in Athena the patient referral status updates and communications.
  • Maintains close communication with referral partners to ensure referrals are received and scheduled.
  • Communicates with patients about offsite referral appointments, including specific details and expectations about the referral.
  • Maintains an accurate specialist and diagnostic referral spreadsheet.
  • Maintains a referral and diagnostic resource list for options.
  • Maintains a clear and efficient referral process for Siloam’s offsite referral programs.
  • Facilitates biannual meetings with staff providers to educate them on offsite referral procedures for diagnostics and specialists.
  • Other duties as assigned by supervisor.
