The Patient and Mediation Safety Officer is an integral member of the IQIPS and Pharmacy leadership team and reports to the Vice President of Quality and the Core Line Administrator for Pharmacy. The Patient and Medication Safety Officer - MCHS provides vision and guidance toward the development of organization culture and experience that prioritizes patient safety. The Patient and Medication Safety Officer - MCHS coordinates and has accountability for the activities, which assure timely reporting of Patient Safety Work Product (PSWP) to the Patient Safety Committee. This individual will actively participate in Patient and Medication Safety learning collaboratives and ensures implementation of best practices, alerts, and updates to drive patient safety improvement. Preferred onsite location Marshfield, WI but other pharmacy locations may be considered.

  • Coordinates and has accountability for the activities, which assure timely reporting of Patient Safety Work Product (PSWP) to the Patient Safety Committee.
  • Actively participate in Patient and Medication Safety learning collaboratives and ensures implementation of best practices, alerts, and updates to drive patient safety improvement.
  • Doctor of Pharmacy Degree.
  • Five years of practice experience with demonstrated growth, knowledge and skills in medication use safety, or completion of PGY1 residency and PGY2 residency in Medication Use Safety.
  • Experience with quality improvement, quality assurance, or quality control procedures and regulatory requirements or substantial knowledge of system processes.
  • Demonstrated ability to work independently and prioritize multiple tasks and produce output with a high degree of accuracy.
  • Demonstrated leadership skills.
  • Demonstrate proficiency in Word, Excel, and Power Point.
  • Licensed as a Pharmacist in the State of Wisconsin or eligibility for WI pharmacist licensure that must be obtained within three months of hire.
  • Certification Professionals in Patient Safety (CPPS) awarded by the Certification Board of Professionals in Patient Safety within twelve months of hire.
  • Continued professional development through completion of ASHP/ISMP Medication Safety certification and Board certification through the Board of Pharmaceutical Specialties must be completed within three years of hire.
  • Vaccine Administration Certificate: Completion of a course of study and training in administration technique conducted by a course provider approved by the Accreditation Council for Pharmacy Education or the board within one year of hire.
  • Employee may not at any time have been or be excluded from participation in any federally funded program, including Medicare and Medicaid. This is a condition of employment. Employee must immediately notify his/her manager or the Health System’s Compliance Officer if he/she is threatened with exclusion or becomes excluded from any federally funded program
  • Master’s degree in related field.
  • Five year’s Pharmacist experience and patient safety management experience in acute or ambulatory care.
  • Certification Professionals in Patient Safety (CPPS) awarded by the Certification Board of Professionals in Patient Safety.
