Position Summary: The Patient Advocate serves as an extension of the Business Development team, supporting the organization’s mission to ensure every patient and family has access to exceptional end-of-life care. This role focuses on community engagement, marketing, and patient coordination support, with a primary responsibility of assisting in relationship-building with referral sources and ensuring smooth communication between facilities, patients, and the hospice team. The Patient Advocate plays a key role in educating healthcare partners and the community about clear hospice services, supporting patient transitions, and ensuring that the referral and consent processes are managed with compassion, accuracy, and professionalism. This position is responsible for promoting a compassionate and credible image to the community for both themselves and ProCare Hospice of Nevada.
