Provides professional spiritual services in accordance with the needs of hospice patients, families, staff, and volunteers. Develops individual spiritual care plans, which demonstrate an effort to in collaboration with local clergy and spiritual counselors. Assesses, implements, an supervises spiritual care in keeping with the patient/family’s belief system Collaborates with the Hospice Interdisciplinary Team in assessing and meeting patient/family’s spiritual and bereavement needs. Develops spiritual and bereavement care plans for each client receiving bereavement support. Provides spiritual care consistent with the physician’s plan of care in clinical record. Includes travel time, visits with patients and families, and documentation Provides religious/spiritual consultation to clients and staff. Spiritual and bereavement care to patient/family at time of death. Coordinates bereavement follow up by Interdisciplinary Team Members and volunteers. Maintains a record of bereavement activities in EPIC Provides direct bereavement support to families through phone calls and visits. Is available to staff as well. Facilitate a bereavement support group. May be required to do On-call spiritual care on evenings, weekends, and Holidays. Work with management, administrative, professional, paraprofessional, staff support, and area clergy. Adheres to standards of Agency’s quality assurance program, develops and maintains a resource group of community clergy to provide specific aspects of spiritual care. Will actively participate in the performance improvement process known as H3W. Participate in hospice support groups. Performs other duties as assigned.

Requirements

  • Bachelor’s degree with training in pastoral care required.
  • One year experience in pastoral care, including experience in counseling the dying and the grieving required.

Nice To Haves

  • Masters level of Theology or Pastoral Counseling Clinical Pastoral Education units preferred.
  • More than one year of supporting the dying and grieving preferred.
