About The Position

The PASRR Specialized Services/Waiver Care Manager is responsible for providing 1:1 specialized services to those who live in nursing facilities and have been approved by the State to receive these services. This position will also become knowledgeable of the EVV (Electronic Visit Verification) system and assist in keeping updated with corrections submitted and other various tasks. Will be eligible to carry a caseload for those participants receiving waiver services.

Requirements

  • BS/BA degree in Psychology, Sociology or Social Work or other Human Services degree approved by DDID.
  • One (1) year experience in working with individuals with intellectual and/or developmental disabilities.
  • Valid driver’s license and the ability to provide safe and reliable transportation.
  • Must possess great organizational skills, time management skills, be able to read and interpret regulations, and have good communication skills (both internally and externally).
  • Physical ability to do light lifting.
  • PASRR Specialized Services Staff/ MPW Assessor/waiver CM is required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually.
  • Complete Case Management Training (conducted by DDID), core training, CPR/First Aid, and Medication Administration/seizure training before beginning independent functioning as a Case Manager.

Responsibilities

  • Provides 1:1 specialized services to those residing in a nursing facility in which the PASRR Evaluator and the Department has determined specialized services are needed. Services for a new participant referred must begin within 30 days of the date specified on the specialized services letter.
  • Works closely with the PASRR Specialized Services Case Manager, and the nursing facility to ensure the needs of each participant are being met.
  • Completes detailed per contact notes for each encounter with participant and turns into supervisor with checklist on a weekly basis. (non-billable and face to face)
  • Attends trainings and keeps updated on PASRR Specialized Services and SGF protocols
  • Becomes knowledgeable and attends video trainings from the EVV provider to learn about the system. Also responsible for attending any trainings related to EVV updates.
  • Works well with staff who are involved in the EVV process and assist with streamlining the current process within River Valley and the process for PDS employees.
  • Will provide Waiver CM services to a caseload. Caseload size will be at Supervisor discretion based upon staff performance. Any case management services performed will be paid at an extra pay rate per case for this positon.
  • Analyzes participant/guardian situations and compiles and interprets data to complete a comprehensive assessment/ Individual Narrative of psycho-social needs for participant.
  • Performs Face to Face contacts and needed consultations with other service providers/professionals who are involved with the overall treatment plan of the participant. Contacts and consultations are performed during work hours set by supervisor based upon client need.
  • Facilitates Team meetings in which treatment plans/ service plans are developed, modified, and reviewed. Development of plans will be based on the client’s assessment and will be person centered.
  • Advocates for the participant/ families interest concerning services, funding sources, treatment and program alternatives, community resource activities, and life choices.
  • Promotes the protection of participant/guardian rights and ensures due process in the event of violation/ restriction of rights.
  • Educates, promotes and encourages participants in area of self-advocacy whenever possible.
  • Completes per contact notes and/or monthly summaries for each waiver participant which serves as an analysis of all services monthly, data, and information. Analysis of data is related to participants outcomes and goals.
  • Identifies and seeks appropriate resources for resolution during a crisis situation. Assists participant/ family to access resources in order to attempt to prevent future crisis.
  • Trains direct support staff to provide needed individualized supports for each participant at various service sites.
  • Provides on-call services to assess, evaluate, and direct staff and/or client to ensure health, safety, and welfare
  • Fosters collaborative working relationships among members of the leadership and management teams and programs.
  • Other duties as assigned
