About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program for individuals 55 years or older, who are living with chronic illness or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. This is a temporary position with an anticipated duration of 3 - 6 months. Hours: Monday - Friday 8:00 am to 4:30 pm.

Requirements

  • High School Diploma or equivalent required.
  • Two years of administrative support experience.
  • Knowledge of organizational policies, procedures, and systems.
  • Knowledge of computer systems and word processing.
  • Skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff, and the public.
  • Skilled in identifying and recommending problem resolution.

Responsibilities

  • Obtain information from participants as directed by providers and/or supervisor regarding referred services.
  • Coordinate overall planning and coordinating of services based on information obtained from participants and team meetings.
  • Schedule participant appointments, including outside consultations, inpatient/outpatient, residential programs, and diagnostic studies.
  • Schedule for CNA and/or registry staffing for assigned departments as needed.
  • Ensure referred service providers have pertinent information to treat or provide services to PACE participants.
  • Notify participants of upcoming appointments and schedule transportation when needed.
