San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. This is a temporary position with an anticipated duration of 6 months. Hours: Monday - Friday 8:00 am to 4:30 pm. Job Summary: The Participant Care Coordinator (PCC) is responsible for scheduling participant appointments across various services which may include medical, dental, therapy, specialty care, and transportation. This role ensures ordered appointments are effectuated as outlined in participants individualized care plans and regulatory requirements. PCCs support the interdisciplinary care team (IDT) in delivering seamless care and are in continuous communication with participants, interdisciplinary staff, and contracted providers to obtain information from participants and from their respective team meetings for overall planning and coordinating of clinical services.
