About The Position

Participant Care Assistant (PCA) - Day Center The P rogram of A ll-Inclusive C are for the E lderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Primary Purpose Under the supervision of the PCA Coordinator, Participant Care Assistants (PCA’s)will provide participants care within the day center, community setting, and in participant homes as directed by your leader or designee in accordance with the participant’s assessment and plan of care.

Requirements

Must possess, as a minimum, a high school diploma, or equivalent.
Well-developed ability to show discretion and maintain confidentiality.
Good communication skills, both written and verbal.
Detail oriented with good problem-solving skills.
Basic computer skills.
Minimum of one year of experience working with the frail, elderly, or long-term care population.
Must have reliable transportation, current valid Michigan driver’s license and car insurance.
Must be 18 years of age or older.
Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
Must be willing to work beyond normal hours and be part of our on-call rotation.

Responsibilities

May assist with delivery of supplies to participants homes as assigned by the PCA Coordinator or designee.
Assist participants with ADLs in the center.
Respects participants autonomy and dignity
Promotes participants to engage at maximum level of independence during activities
