Conduct an admission interview with the resident and family for the purpose of completing a confidential psychosocial history and assessment, identify areas of concern, develop treatment plans/goals, and complete advance directives. Update residents’ charts/social services records every three months following initial evaluation and/or as social services needs are demonstrated. Re-evaluate resident care plans at least every three months. Assisting with scheduling transportation. Send bereavement cards and notify families or responsible parties when residents expire and document. Contact families to pick up personal belonging for resident who return to the hospital. Handle ancillary services for the residents (dental, ent, optometry, psychology consult, ophthalmology). Oversee room changes - completing acknowledgement form, document, and notify families. Attend weight and wounds meetings and complete the social service section of assessment for weight variance meeting. Work on the weekends if able. Theft & loss and grievances. Conduct Angel rounds & IP rounds. Assist with discharge planning. Contribute to Resident Care Conferences, and communicate resident and family areas of concern.
