About The Position

The Home Infusion Nurse at Optum is responsible for providing clinical care and education to home-based patients receiving parenteral medications. This role involves administering medications, establishing vascular access, and collaborating with a multi-disciplinary team to ensure safe and effective therapy delivery. The position requires working in patients' homes and ambulatory infusion suites, focusing on improving health outcomes and advancing health equity.

Requirements

  • Active and unrestricted licensure as a Registered Nurse (RN) in the state(s) of practice.
  • Current Basic Life Support (BLS) and/or CPR certification, or willing to obtain.
  • 1+ years of experience working as an RN in a clinical care setting that contributes to proficiency in infusion therapy.
  • Proficient with electronic medical records documentation.
  • Ability to work required schedule of Wednesday, Thursday, Friday, Saturday 12:00pm-7:00pm.
  • Willing and able to work independently in home or alternate-site settings.
  • Access to reliable transportation for travel to customer and/or patient sites.

Nice To Haves

  • Experience administering infusion therapy in a home or alternate-site setting.
  • Proven knowledge of and experience with Microsoft Office and electronic medical records (EMR) programs.
  • Proven knowledge of the Infusion Nursing Society (INS) standards of practice.
  • Proven knowledge of HIPAA patient privacy and data security requirements.
  • Proven effective interpersonal communications, both written and verbal.
  • Proven effective teaching skills.
  • Proven excellent organizational and critical thinking skills.

Responsibilities

  • Establish and maintain positive clinical rapport with patients and their families.
  • Assess patient appropriateness for home infusion therapy, including safety of the home environment and caregiver support.
  • Obtain and document patient's medical history at the start of care and ongoing.
  • Complete all required clinical documentation thoroughly and timely.
  • Participate in case conferences to plan patient care with the multi-disciplinary care team.
  • Provide skilled care to patients of varying ages and medical conditions as outlined in the plan of care.
  • Establish, secure and maintain vascular access devices as prescribed.
  • Administer medications as prescribed, performing therapy-specific clinical assessments.
  • Communicate observations of patient response to therapy with members of the care team.
  • Establish collaborative relationships with home health agencies delivering services.
  • Maintain a comprehensive knowledge base of infusion therapies administered in the home and ambulatory infusion center.
  • Maintain current nursing licensure in all states in which patients are serviced.
  • Comply with all State and Federal regulations and laws.
  • Complete all training required upon hire and annually per policy.
  • Serve as a professional representative of the Company when providing education to external customers.
  • Assist in staff orientation and training.
