Requirements

• Valid RN License

• Must have passed the NCLEX-PN exam
• Willingness to travel.

Responsibilities

• Assess and chart observations of the patient's condition at each visit.
• Complete evaluation tasks, including reviewing medication and specific plan of care.
• Coordinate with Physicians, Nurse aids, and other individuals in the patient's care plan.
• Recommend tools or devices that may improve the quality of life for the patient.
• Submit assessments to the office in a timely manner.
