Requirements

• Current Michigan License in good standing
• Empathetic approach to patients and caregivers
• Ability to adapt to patient’s status and needs on a full time on call basis

Nice To Haves

• Experience in private duty nursing
• Ability to participate in field visits daily and weekly office meetings as indicated

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Document all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record
• Supervision of paraprofessionals providing services to patient
• Maintain high level of customer satisfaction with our clients, families and caregivers
