About The Position

Position Summary Fora Health is seeking a mission driven addiction medicine physician to join our expanding medical services team in a part time role, providing direct patient care across programs and supporting the medical department leadership team with program development as needed. This physician will join a collaborative, interdisciplinary team of addiction medicine physicians, advanced practice providers, nurses, case managers and peer recovery mentors in providing compassionate, patient centered care to people who use substances.

Requirements

  • Must be a licensed MD or DO
  • Must be board certified in Addiction Medicine; Fellowship training strongly preferred
  • Experience in prescribing buprenorphine required
  • At least one year of attending experience in primary care, hospital medicine or urgent care required
  • Experience in withdrawal management strongly preferred
  • Verification of Sobriety, per Oregon Administrative Rules (OAR) standards
  • DHS Background Check Approval or alternate Criminal History Check agency approval
  • Successful completion of Drug Test upon hire
  • Documentation of Tuberculin test and/or evaluation with negative results or evidence of non-communicability

Responsibilities

  • Complete medical intakes for patients admitted to Withdrawal Management and residential programs.
  • Develop and document individualized treatment plans consistent with evidence-based practice, harm reduction principles, and patient goals of care.
  • Provide ongoing direct patient care in residential programs in collaboration with nursing, psychiatric, case management, and peer staff.
  • Conduct both in person and telehealth visits for MAT patients, including prescribing medications, completing follow ups, and administering or supervising injectable treatments.
  • Review medical referrals and communicate with referring hospitals, outpatient clinics, and community partners regarding patient suitability for WM and Residential services.
  • Participate in rotating on call coverage supporting onsite nursing staff, typically one week per month or as scheduled with the CMO.
  • Communicate clearly with interdisciplinary team members regarding medical needs, treatment changes, and patient progress.
  • Partner with the Chief Medical Officer, Director of Psychiatric Services, Medical Director and Medical Operations Director as needed to design, pilot, and refine new medical programs within WM, residential, and outpatient settings.
  • Assist in expanding MAT services, including development of programming for PAWS management, tobacco cessation, and alcohol use disorder pharmacotherapy.
  • Contribute to workflow design, including EHR order sets, standard work documents, policies and procedures, and clinical decision pathways.
  • Support initiatives that strengthen rapid access from local hospitals and community partners into Fora medical services.
  • Participate in quality improvement initiatives, data review, and outcome monitoring to enhance patient care and program efficiency.
  • Support development of patient education curricula and resources as needed.
  • Work collaboratively with medical, psychiatric, nursing, and organizational leadership, including ongoing partnership with the Director of Psychiatric Services to support integrated care across programs.
  • Attend medical team meetings, interdisciplinary case reviews, and supervision sessions as assigned.
  • Provide mentorship and clinical experience to medical students, residents, and fellows rotating through Fora Health.
  • Model harm reduction, trauma informed care, motivational enhancement techniques, and loving-kindness in all patient and staff interactions.
  • Maintain strong, proactive communication with colleagues across departments to ensure continuity of care and program alignment.
  • Attend and participate in all staff meetings, including medical department and leadership meetings as assigned.
  • Attend all supervision meetings as assigned.
  • Maintain and strengthen good working relationships with members of other departments.
  • Maintain cleanliness and safety of work environment.
  • Perform related tasks as assigned.
  • Complete documentation in a timely manner.
  • Negotiate conflicting demands of patients, staff, available resources, and applicable policies & laws.
  • Prioritize and manage the competing demands of their work effectively.
