Part-Time Physician – Post-Acute Care - Southern, NJ
About The Position
We are seeking a compassionate and skilled part time Post-Acute Care Physician to join our care team. In this role, you will support patients and families as they transition from hospital to post-acute settings, overseeing recovery and managing complex medical needs with clinical expertise and empathy. The Physician will provide comprehensive medical evaluation and management for patients in post-acute settings, including skilled nursing and rehabilitation facilities. This role offers the opportunity to build meaningful relationships with patients and families while guiding them through critical stages of recovery.
Responsibilities
- Leading and coordinating post-acute medical care plans
- Managing acute and chronic conditions during recovery
- Providing symptom management and goals-of-care discussions
- Collaborating with interdisciplinary teams, including nursing, therapy, and case management
- Supporting safe transitions of care and reducing readmissions
