About The Position

Elderberry Health (Circle of Life Clinic) is a Rural Health Clinic serving the Ashland City community and surrounding areas. We provide comprehensive primary care, women’s health, chronic disease management, and wellness services. Our team focuses on high-quality, patient-centered care in a collaborative and supportive environment. We are seeking a part-time Family Medicine or Internal Medicine Physician to join our growing primary care team. This position is ideal for a clinician who values continuity of care and enjoys working in a rural outpatient setting with a focus on preventive medicine and chronic disease management. You’ll see a manageable volume of patients in a well-supported clinical environment with experienced nursing and administrative staff. No hospital call, weekends, or after-hours duties.

Requirements

• MD or DO with training in Family Medicine or Internal Medicine.
• Active Tennessee medical license (or eligibility for licensure).
• Current DEA registration.
• Strong communication, teamwork, and patient relationship skills.

Nice To Haves

• Experience with Rural Health Clinic operations or value-based care preferred but not required.

Responsibilities

• Provide comprehensive outpatient primary care, including preventive care, chronic disease management, and acute visits.
• Collaborate with nursing, behavioral health, and care coordination teams to ensure integrated patient care.
• Utilize eClinicalWorks (ECW) EHR for documentation and care management workflows.
• Participate in quality improvement and value-based care initiatives.
• Provide supervision and support for nurse practitioners as needed (if applicable).
