About The Position

Empyrean Hospice hires Licensed Practical/Vocational Nurses to provide direct patient care under the supervision of a registered nurse. The position requires representing Empyrean Hospice with professionalism and compassion, providing direct patient care as defined in the State Nurse Practice Act, and working collaboratively with the interdisciplinary team to achieve positive patient care outcomes. Responsibilities include assessing and providing education to patients and families, maintaining accurate clinical notes, ensuring a safe environment, and prioritizing ethical standards in clinical practice.

Requirements

  • Professionalism, compassion, empathy, and a desire to help others
  • Self-directing with the ability to work with little direct supervision, yet also as a member of a complete interdisciplinary care team
  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years nursing experience; community health/hospice or medical/surgical experience is preferred
  • Currently licensed as an LPN/LVN in the State and complies with accepted professional standards and practice
  • Demonstrates good verbal and written communication, and organization skills
  • Possesses and maintains current CPR certification
  • Able to lift, position and/or transfer patients, and able to lift supplies and equipment
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Responsibilities

  • Represent Empyrean Hospice with the utmost professionalism and compassion
  • Provide direct patient care under the supervision of a registered nurse and as defined in State Nurse Practice Act, including specialized duties related to end of life care
  • Work collaboratively with the members of the interdisciplinary team to help meet positive patient care outcomes
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and the plan of care
  • Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given
  • Provide and maintain a safe environment for the patient and arrange equipment and other necessary items and services as needed
  • Ensure standards of ethical business and clinical practice are prioritized
