The Opportunity (Part-Time) We’re hiring a part-time LVN to lead in-field medication administration for Medication-Assisted Treatment (MAT)—with a focus on long-acting injectables (e.g., buprenorphine and naltrexone) under prescriber orders. You’ll pair excellent nursing fundamentals with clear, timely documentation and tight coordination with providers and Community Health Workers (CHWs) to keep patients engaged and on schedule.

Requirements

  • Active California LVN license in good standing.
  • BLS required.
  • Valid CA driver’s license, reliable insured vehicle, and ability to travel throughout LA County.
  • Organized self-starter who can independently plan your day (routing, timeboxing, contingency plans) to reach as many patients as possible—without sacrificing safety or quality.
  • Comfortable providing direct care outdoors/in non-traditional settings under dynamic, unpredictable conditions; you stay calm and solutions-oriented.
  • Strong communicator with a trauma-informed, harm-reduction mindset; collaborative with CHWs, RNs/NPs/PAs, and partners.
  • Fluent documentation skills; you keep charts current and crisp so the team always has the latest picture.
  • Experience (or strong interest) working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, individuals transitioning from incarceration, and adults with complex medical needs).

Responsibilities

  • Administer MAT injections and long-acting injectables in the field per standing orders/prescriber direction; monitor for adverse events; document lot, site, and response in real time.
  • Own your day plan during scheduled work hours: independently plan routes, stack visits, and manage time to maximize completed nursing visits and on-time injections; adjust dynamically as patient needs change.
  • Perform focused nursing visits in street settings: vitals, symptom/side-effect checks, basic wound care, withdrawal/COWS monitoring, vaccination, and patient education.
  • Manage cold chain & supplies (secure transport, temperature logs, inventory, kit readiness, sharps handling/disposal).
  • Coordinate with prescribers, CHWs, and pharmacies to confirm eligibility, arrange deliveries/pickups, and re-engage patients if doses are missed.
  • Document thoroughly in Akido systems (EHR/case-management platform) to maintain continuity across the multidisciplinary team.
  • Promote harm reduction & safety (naloxone education/distribution; escalate per protocol; support transitions of care).
  • Participate in brief huddles/case conferences as scheduled, contribute to quality improvement, and update protocols for field-based MAT delivery.
  • Other duties as assigned.
