We are seeking a dedicated Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care to patients in their homes on an intermittent basis. This role operates under the direction and supervision of a Registered Nurse (RN), following physician orders and care plans in line with our Agency's policies.

  • Adhere to established policies and procedures.
  • Implement nursing care plans for each patient.
  • Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.
  • Initiate preventive and rehabilitative nursing procedures as appropriate.
  • Observe signs and symptoms, reporting reactions to treatments to the physician and RN.
  • Teach and counsel patients and their families regarding nursing care needs and related problems at home.
  • Evaluate the effectiveness of the LPN/LVN's nursing service with the RN.
  • Maintain accurate and complete records of observations, treatments, and patient care.
  • Participate in medical record audits as assigned.
  • Attend staff meetings, patient care conferences, and in-services as scheduled.
  • Take on-call duty, including nights, weekends, and holidays as assigned.
  • Submit schedule changes to the Director of Clinical Services/Nursing Supervisor daily.
  • Participate in patient care conferences for team collaboration.
  • Prepare clinical and progress notes.
  • Assist physicians and RNs in performing specialized procedures.
  • Prepare equipment and materials for treatments.
  • Observe aseptic technique as required.
  • Assist patients in learning appropriate self-care techniques.
  • Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state(s) of practice.
  • Minimum of one (1) year of nursing experience preferred.
  • Alignment with the philosophy and goals of our Agency.
  • Ability to exercise initiative and independent judgment.
