The Health Equity Action Team HEAT Social Worker will work with an interdisciplinary team of physicians, physician assistants, and peer health workers to provide enhanced services and support for patients with HIV or substance use disorder. Under the supervision of the Section Chief of Infectious Diseases, the Social Worker will provide psychosocial assessment and counseling services to hospitalized patients with the primary goal of ensuring that they are able to attend their initial outpatient follow-up clinic appointment.
