About The Position

The Part-time IRIS Consultant Case Manager is responsible for providing ongoing support and assistance to participants enrolled in the IRIS program in Southwest Wisconsin. This role focuses on helping participants self-direct services to meet their long-term care goals and life outcomes through a community-based network of supports. The consultant will conduct home visits, complete necessary documentation, and collaborate with various stakeholders to ensure participants receive the appropriate services and support.

Requirements

  • Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field preferred, or a high school diploma with additional experience.
  • One year of experience in delivering social services to target groups (intellectual disabilities, physical disabilities, and frail elders) with a bachelor's degree, or four years of relevant experience without a degree.
  • Valid Wisconsin driver's license and auto insurance if utilizing a personal vehicle.

  • Knowledge of community resources and disability service providers in the region.

Responsibilities

  • Complete home visits and phone contacts to assist participants with self-directing services and ensure their health and safety.
  • Travel regularly within a three-hour radius and farther distances as needed to provide coverage for other consultants.
  • Accurately complete Individual Support and Services Plans (ISSP), Critical Incident Reports (CIR), and other required documentation as per IRIS program policies.
  • If qualified, complete the Long Term Care Functional Screen (LTCFS) certification and annual rescreens for participant caseload.
  • Direct collaboration between participants, natural supports, service providers, and community resources to assist participants in meeting their life outcomes.
  • Assist other consultants during absences and provide training to new consultants as needed.
  • Maintain financial eligibility for participants by tracking Medicaid reviews and assisting with necessary paperwork.
  • Attend scheduled phone conferences and team meetings as required by the Regional Supervisor.
  • Participate in all required orientation and training sessions.
  • Partner with the quality department to develop measures supporting program outcomes.
  • Perform special assignments and projects as assigned.
