About The Position

This is a part-time position for a Family Child Health Nurse to provide home visiting care in the community. The RN in this position would support both the Babies First! And CaCoon Programs. This role is best suited for a nurse with strong interpersonal communication and case management skills. This RN will build and maintain a client caseload who they will support in goal setting, life-course progression, health education, and skill building. Therapeutic communication, empathic listening, boundary setting/maintenance, personal self-care, comfort with home visiting, and the ability to work with families from all walks of life are key to success in this role. Position Summary Performs a full range of duties including planning, implementing and delivering community health nursing such as assessment, care delivery and counseling; and, maintains records, manages program finances, maintains supplies and performs other administrative tasks as necessary. Performs work independently except as new or unusual circumstances require; performs the most difficult and responsible types of duties assigned to classes within this series.

Requirements

  • Oregon RN license required.
  • Pass a criminal background check prior to hire.
  • Submit an acceptable DMV certified court print prior to hire.
  • Pass a pre-employment drug screen, which includes screening for Marijuana.
  • Requires possession of a valid driver's license and an acceptable driving record.
  • Must be currently licensed by the state of Oregon as a registered nurse.
  • Must have CPR certification.
  • Bachelor's degree in nursing or healthcare administration AND two years of public health nursing experience; OR
  • An Oregon RN with a Bachelor's degree in a field other than nursing or healthcare administration AND four years public health nursing experience; OR
  • Any combination of education and experience which provides the applicant with the desired skills, knowledge and ability required to perform the job.

Responsibilities

  • Assesses individual and family health status; identifies health problems and perform advanced nursing care in a variety of settings, often in individual's homes; conducts comprehensive health assessments; demonstrates in-depth clinical judgment and skills within the scope of practice as permitted.
  • Responds appropriately to emergency medical situations.
  • Maintains accurate and up-to-date patient records and health program statistics; analyzes community health trends affecting assigned program or caseload; prepares records and reports.
  • Plans and implements nursing care in a community health program, coordinating service delivery within assigned program area; assists with the implementation, coordination and planning of new nursing projects; evaluates services on an on-going basis and recommends methods of improvement as necessary.
  • Develops health education plans and teach and/or counsel individuals, families and groups about disease containment, treatment and prevention, family planning, nutrition, general health and other issues affecting general health at the complex level; provides health education and outreach.
  • Works with other agencies regarding health care issues; acts as consultant to health care providers on community health issues in assigned program area; may serve on committees, boards and task forces as appropriate.
  • Reviews and oversees quality control standards for patient care and ensure compliance with the standards.
  • May provide oversight and/or leadership of subordinate staff members, volunteers, interns or other aides as assigned; assist in training staff members.
  • Adheres to regulatory requirements, collective bargaining agreements, County and departmental policies and procedures, required trainings, as well as safe work practices.
  • Develops and maintains effective internal and external working relationships at all levels.
  • Has regular and reliable attendance. Overtime may be required.
