St. Joseph's Medical Center is seeking a trained peer specialist to fill the part-time Care Coordinator Assistant position in our Care Coordination Department. Qualified candidates will have a peer specialist training certificate and a NYS driver's license. Candidates currently enrolled in a peer specialist training program will also be considered. The Care Coordinator Assistant provides recovery-oriented services in the community to clients with medical and behavioral health challenges. Services are designed to assist clients achieve their goals, particularly in the areas of independent living, access to healthcare, and building a support system in the community. Candidate must drive, have a valid driver's license and their own vehicle.
