The Part-Time Assessment RN (UAS) position at Healthfirst involves conducting in-person and virtual assessments for prospective members and re-assessments of current members using the Uniform Assessment System (UAS). The role requires the nurse to utilize their clinical expertise and critical thinking skills to make enrollment and continued eligibility determinations. Assessments may take place in various settings, including skilled nursing facilities, and the nurse will also conduct assessments in the home environment as needed. In addition to assessments, the nurse will complete evaluations of service needs based on requests from the Care Management Team (CMT) and the member's family or caregivers. This includes reviewing previous nursing assessments and medical notes to determine necessary adjustments in the Care Plan or to assist with nursing home admissions. The nurse will also conduct Care Management visits to support person-centered service planning and will act as a liaison between the member and the CMT, assessing the home environment and psychosocial status. The role also involves providing feedback to home care agencies and the CMT regarding home care issues and the performance of contract nurses and aides. The nurse will be responsible for community-based training for the frail population, focusing on self-care techniques and prevention strategies. Ensuring that prospective members and their significant others understand and agree to enrollment in a managed long-term care plan is also a critical part of the job. The nurse will manage scheduling activities to ensure that prospects, members, and caregivers are informed of their scheduled visits and will participate in orientation, training, and meetings as required. Additional duties may be assigned as necessary.
