About The Position

Clover Health is seeking a Part-Time Telehealth Practitioner to join their clinical team, dedicated to improving the lives of Medicare Advantage members through remote health assessments and care coordination. This role involves conducting comprehensive health assessments, creating personalized care plans, and ensuring effective communication with interdisciplinary care teams, all while utilizing technology to deliver patient-centered care.

Requirements

• Valid RN License in New Jersey.
• Current provider card in Basic Life Support (BLS).
• Minimum of 1 year clinical work experience providing direct patient care.
• Bachelor's degree (BSN) from an accredited school of nursing or equivalent qualifications for MD/DO.
• Master's degree in advanced clinical practice nursing or graduate of post Master's certificate program for Nurse Practitioner or an MD/DO.
• Active DEA and CDS registration.

Nice To Haves

• Certification as an Advanced Nurse Practitioner in specialty areas or MD/DO from an accredited program.
• Excellent clinical skills with knowledge of long-term care, geriatrics, and chronic illness.
• Strong customer experience skills.
• Tech-savvy with comfort in using electronic health data and telehealth technology.

Responsibilities

• Deliver preventive care to Clover members remotely using Clover's proprietary software and approved third-party video apps.
• Complete comprehensive reviews and assessments of members' medical histories, physical health, environment, and psycho-social status.
• Document progress notes, prepare member workups, and encounter summaries using Clover Assistant and EMR.
• Assist at-risk members in transitioning from hospitals and other facilities to their homes.
• Create personalized care plans in coordination with the member's primary care physician.
• Actively close gaps in care, including ordering labs, writing prescriptions, and coordinating follow-ups with physicians.
• Perform point of care labs and diagnostic testing, such as A1c and spirometry.
• Ensure accurate documentation of member health conditions and compliance with Clover Care Service policies and procedures.
• Participate in ongoing training and meetings remotely.
