Northwest HealthCare, six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services. The Emergency Medical Services Department is charged with the delivery, management and transport of patients involved in traumatic and medical emergencies. A Mobile Integrated Health (MIH) paramedic has a comprehensive medical knowledge base and is able to provide post hospitalization care for a broad range of medical conditions. The MIH paramedic will also be able to provide advanced level medical care including; post discharge evaluations, primary care in the home, and Social Determinates of Health. Must have Illinois paramedic license and IDPH MIH program completion.

Responsibilities

  • Conducts post-hospital release follow-up care including, but not limited to, monitoring medication, dressing changes, and checking vital signs.
  • Conducts physical exams in patient's homes and works with physicians to provide continuity of care.
  • Observes, records, and reports to physician, patient’s conditions and reactions to drugs, treatments, and significant incidents.
  • Works autonomously and in the best interest of the patient.
  • Conducts follow-up visits to post-discharged patients to provide necessary connection to community resources and primary care while monitoring medical needs.
