About The Position

ChristianaCare is seeking experienced and innovative Paramedics to fill the position of Mobile Integrated Health Community Paramedic. This position will work within the ChristianaCare Center for Virtual Health, Hospital at Home program. The Community Paramedic is an integral part of the ChristianaCare care team. The paramedic provides in-home patient assessments, clinical and social screenings, clinical interventions, patient education, and participates in telehealth visits from the patient's home while connecting with the physician or virtual command center via Telehealth. Mobile Integrated Healthcare is the provision of healthcare using patient-centered, mobile resources in the out-of-hospital environment. At ChristianaCare, we offer Hospital at Home, which provides hospital-level care in the patient's home. Hospital at Home is designed to enhance a patient's experience by providing comprehensive, safe and efficient in-home care as an alternative to hospitalization.

Requirements

  • BLS, ACLS, PALS Certifications required.
  • National Paramedic Licensure required upon hire.
  • Delaware, Pennsylvania, and Maryland licensure to be obtained and maintained as needed.
  • Willingness to obtain licensure in additional states as program needs arise.
  • Community paramedic certification must be acquired within 24 months of employment.
  • EVO-C Certification and maintain a driving record in compliance with ChristianaCare policy.

Responsibilities

  • Provide in-home patient assessments and clinical screenings.
  • Conduct clinical interventions and patient education.
  • Participate in telehealth visits connecting with physicians or virtual command center.
  • Work within the ChristianaCare Center for Virtual Health, Hospital at Home program.
