About The Position

UW Medicine Primary Care and Population Health has an outstanding opportunity for a Patient Services Specialist 3 - Panel Navigator. WORK SCHEDULE Full-time, 100% FTE (40 hours per week) Monday-Friday After successful completion of 6 months probationary period- 2 days / week of remote work available Day Shift DEPARTMENT DESCRIPTION The Madison Clinic at Harborview offers medical care and social services for persons living with HIV/AIDS, regardless of sexual orientation, gender identity, race or ability to pay. Our mental healthcare professionals provide a range of services for patients with HIV/AIDS, including psychiatric consultations, medication management and recommendations for continued psychiatric care. POSITION HIGHLIGHTS The Madison panel navigator has to work with the satellite Madison clinics at Snohomish, Kent and Federal Way as well as the downtown programs (Pioneer Square clinics). They will assist in coordinating population health programs at HMC primary care clinics and support leadership communications for population health initiatives. Independently provide support services such as processing incoming referrals, scheduling patient appointments, obtaining and verifying demographic and financial information through pre-registration process, completing patient registration by obtaining consent for medical care and financial agreement, and processing patient paperwork and coordinating care with other services, and triaging patient telephone calls to clinical staff and providing information about services. Requirements

  • High School graduation or equivalent
  • Two years Patient Services Specialist 2 experience OR An equivalent combination of education/experience

Responsibilities

  • Engage the patient in care optimization
  • Empower the patient with knowledge and access to support resources.
  • Improve the patient and family experience of care.
  • Participate in Patients Are First measures and associated Medical Home goals and objectives.
  • Referrals tracking to internal and external resources.
  • Contact relevant community agencies to increase knowledge of services available for patients and families and help eliminate barriers to care.
  • Educate per protocol.
  • Assist in the utilization of culturally relevant and language appropriate materials for target populations.
  • When indicated through population review, provide outreach and assistance with patients/families in the coordination of various wellness and chronic disease activities in collaboration with the health care team.
  • Other duties as requested.
