About The Position

The Palliative Oncology Social Worker II, in outpatient palliative care, assesses patient/family needs and coping skills that support patient#s unique cultural and spiritual beliefs. Provides holistic patient/family centered care across the treatment continuum. Provides education, clinical interventions, and community resources specific to the patient/family need. Serves as member of the patient#s multidisciplinary care team and conducts advance care planning conversations. Coordinates with the care team and other coworkers to facilitate the patient#s goals and high-quality care, focusing on the patient as the center of services

Requirements

  • Must have a Master#s degree in Social Work (MSW)
  • Must have current Indiana LSW or LCSW license
  • Minimum of one (1) year experience in health care
  • Demonstrates thorough working knowledge of social work process, appropriate clinical interventions, community agencies, services, and resources available to assist patients and families with emotional and social difficulties related to change in patient#s health status
  • Requires general knowledge of the normal growth, development, related behavioral, needs, comfort and range of treatment appropriate to the age of the patient and diagnosis to support the provision of an individualized plan of care
  • Requires sound analytical and organizational skills to assess patient#s needs, plan and coordinate appropriate therapeutic interventions and to manage caseload to achieve efficiency and productivity
  • Must have good verbal and written communication skills

Nice To Haves

  • For those employed to work in the outpatient palliative care team, Oncology Social Worker # Certified (OSW-C) or Advanced Palliative Hospice Social Worker # Certified (APHSW-C) preferred
  • experience in palliative and/or hospice health care preferred
  • Prior clinical and/counseling experience preferred
