Palliative Nurse RN
About The Position
In this role you will... Deliver compassionate, comprehensive care by educating patients, managing medications, assessing health, and providing emotional support to patients and families. Coordinate interdisciplinary care plans by collaborating with nurse practitioners, social workers, and other professionals, while triaging patient needs and guiding advance care planning. Ensure seamless transitions and compliance by supporting intake and referrals, maintaining accurate documentation, and facilitating hospice care when appropriate. Travel in all weather conditions to patients homes in the assigned territory.
Requirements
- Valid, unrestricted Iowa driver’s license
- Current vehicle insurance
- Current license as a registered nurse in the state of Iowa
- Two years of full-time nursing experience required

- One year of hospice experience preferred
