About The Position

The Palliative Care Nurse Practitioner provides outpatient palliative care to patients with serious or complex illness, focusing on symptom management, advance care planning, and longitudinal goals‑of‑care discussions. This role collaborates with an interdisciplinary team and referral partners to support coordinated care, hospice readiness, and appropriate transitions across care settings.

Requirements

  • Licensed Nurse Practitioner with national certification
  • State licensure and DEA registration (as applicable)
  • Hospice and Palliative Care board certification required (ACHPN)

Responsibilities

  • Assess and manage pain and other complex symptoms in an outpatient setting
  • Conduct advance care planning and goals‑of‑care discussions
  • Prepare patients and families for hospice readiness and initiate hospice referrals when appropriate
  • Participate in interdisciplinary team meetings and care conferences
  • Collaborate with primary care providers, specialists, and community referral sources
  • Coordinate care across outpatient, hospital, home health, and hospice settings
