About The Position

The Palliative Medicine nurse works with the interdisciplinary Palliative Medicine team to ensure that patients with life-limiting illness receive symptom-directed care that is within established standards of care, and that patients and their families receive information and support that enables them to receive care that is aligned with their values and goals. While the focus of the nurse's work is set in the outpatient Palliative Medicine clinic, work may also be performed in the inpatient setting as needed.

Requirements

  • Bachelor's degree in Nursing (BSN) required prior to or within 5 years of employment
  • Licensure as a Registered Nurse in the State of Washington
  • Current Healthcare Provider BLS - American Heart Association by start date
  • 3 years of previous RN experience
  • Chronic care management and patient education skills
  • Ability to work independently and prioritize multiple responsibilities

Nice To Haves

  • Master's degree in nursing
  • Experience in Palliative Medicine and/or Hospice
  • Experience in quality improvement (QI), advance care planning (ACP), and nursing education
  • Previous experience demonstrating effective, positive and constructive feedback

Responsibilities

  • Participates as a member of the interdisciplinary Palliative Medicine team, and attends weekly team meetings and monthly business meetings
  • Provides clinical support to medical provider(s) in the outpatient Palliative Medicine clinic
  • Serves as a liaison to ensure continuity of care among the Palliative Medicine clinic and hospitals, EDs, specialists, home health programs, hospice programs, and long-term care facilities
  • Triages patients in person and via phone
  • Works with medical providers to develop and implement each patient's plan of care
  • Provides education regarding Palliative Medicine and end-of-life care to patients, families, medical providers, clinical staff, and the community
  • Provides clinical oversight of the Palliative Medicine Coordinator
  • Maintains and analyzes program metrics and QI projects in collaboration with Medical Director
  • Performs nursing visits and serves as a consultant to patients and caregivers
  • Willingness to become credentialed within 6 months of date of hire in order to facilitate Advance Care Planning (ACP) workshops virtually and in person, in both individual and group settings
  • Provides back-up coverage for the Palliative Medicine Coordinator
  • Assists with prior authorizations for medications
  • Performs other duties as assigned
