The Physician Services Palliative Clinical Coordinator is responsible for following through on all Palliative provider orders and needs, patient visit notification, medical record management, diagnostic test ordering and results management, and aiding patient’s caregivers, home health agencies, hospices, post-acute providers, pharmacies and other medical providers. Responsibilities are completed with limited supervision and may involve medical and social service activities and correspondence. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population. In this role you will be responsible for: Coordinate provider orders including home healthcare, hospice services, medical equipment, pharmacy services, and diagnostics Serve as the primary communication link between patients, families, providers, and healthcare agencies Manage medical records and ensure seamless information flow to primary care physicians Answer patient and family questions with patience and expertise Triage patient needs and communicate urgently with providers Navigate communication barriers common in geriatric care, such as hearing loss and cognitive impairment Connect patients and caregivers with health and social service resources Exercise initiative and creative problem-solving in dynamic situations Make independent decisions that prioritize patient wellbeing Adapt quickly to changing priorities while maintaining exceptional service
