About The Position

As a Full-Time Palliative Care Social Worker at AccentCare, you will join a purpose-driven, collaborative culture that sets the standard for excellence in patient care. Requirements

• Master's Degree from an accredited school of Social Work or related field.
• Valid license/registration of permit to practice in state(s) of agency operation if required.
• At least one (1) year of professional social work experience in a healthcare setting required; 2+ years preferred.
• Experience or education in grief counseling preferred.
• Discharge planning experience preferred.
• Must be a licensed driver who can travel to all business locations.

Nice To Haves

• Experience with Medicare/Medicaid/Insurance specialty.

Responsibilities

• Serve as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources.
• Provide counseling to help patients understand their disease trajectory towards acceptance.
• Act as a liaison between patients and the Palliative Care and Hospice teams to ensure continuity of care.
• Assist the Palliative Care team in understanding significant social, spiritual, and emotional factors related to the patient's health.
• Assess the social, spiritual, and emotional needs of patients and caregivers.
• Support patients and families through education and facilitate the completion of Advanced Directives.
• Provide discharge planning related to changes in level-of-care.
• Educate patients and families on managing symptoms and responding to crises.
