Requirements

  • Licensed Clinical Social Worker (LCSW).
  • Advanced Certified Hospice & Palliative Social Worker (ACHP-SW) certification or ability to take exam within 24 months of being hired.
  • Basic Life Support (BLS) through the American Heart Association.

Nice To Haves

  • Minimum of two (2) years working in a direct clinical/therapeutic capacity.
  • Minimum of two (2) years working with life-threatening conditions, chronic disease, palliative care and/or hospice, end-of-life care, and related situations.
  • Knowledgeable of a full range of specialized and expert clinical social work/counseling services for patients/families receiving palliative care.
  • Able to provide crisis intervention, short-term and family therapy.
  • Offers information and referral and/or tangible assistance.
  • Organizational and prioritization skills while determining severity of patient condition and prognosis.
  • Excellent verbal and written communication skills.
  • Basic computer skills.
  • Medical Social Worker experience.

Responsibilities

  • Perform a comprehensive patient and family biopsychosocial-spiritual assessment.
  • Engage with patient and family to facilitate trust, rapport, and communication.
  • Provide brief supportive counseling, referral for crisis intervention, or refer to therapeutic supports as needed.
  • Develop a treatment plan based on the assessment, incorporating counseling referral, advance care planning, and educational components.
  • Act as liaison to promote discussion between the patient, family, and medical team.
  • Provide education and guidance to the patient, family and care team members pertaining to palliative and end-of-life care.
  • Refer the patient and family to appropriate supports and resources.
  • Advocate for the patient and family to ensure respect for patient autonomy and maximize the safety of the patient's care plan.
  • Make recommendations for care plan and disposition if a palliative care consultation is requested.
  • Act as part of the larger Palliative Care Team, communicating clearly and effectively with other team members.
