About The Position

The Palliative Care RN is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, and for promoting the Compassus philosophy using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. Under the supervision of the Director of Clinical Services, the Palliative Care Registered Nurse provides services to palliative care patients and their families following nursing processes and in accordance with the Nurse Practice Act. The Nurse participates as a member of the interdisciplinary team, providing clinical support, communication, and coordination of care across settings. The Palliative Care Nurse functions as an IDT member to provide telephonic and telehealth routine and emergency assessment, educational, and evaluative services to meet the needs of patients and their families. S/he assists the IDT in the assessment and management of the patient's prognosis and symptom burden, and helps maintain their maximum level of comfort and coping. The RN will also participate in evaluating palliative care patients for transition to hospice programs. Major Challenges: Working in a growing program with the associated uncertainties and adapting to new requirements or innovations. Functioning across a variety of settings within the community, such as pharmacies, private homes, assisted living facilities, and the program office. Practicing in an interdependent team environment rather than independently. Being open to rapidly changing patient conditions, preferences, and goals of care, which may require a flexible and creative approach. Patient Population Served: Adult, Geriatric The employee must be competent in interpreting a patient’s self-report or behavior, considering cognitive, physical, emotional/psychosocial, and developmental factors. Treatment should be individualized to reflect the patient’s specific needs.

Requirements

  • Associate’s Degree in Nursing
  • California Registered Nurse License (Vendor Managed)
  • National Provider BLS - American Heart Association (Vendor Managed)
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Strong communication skills including effective verbal and written skills.
  • Ability to relate health care issues to the public.
  • Ability to develop positive working relationships with diverse groups and individuals.
  • Ability to prioritize work.

  • Reliable transportation.

Nice To Haves

  • Bachelor’s Degree in Nursing
  • California: Required – 2 years RN experience
  • HPNA Certification (eligibility for certification required)

Responsibilities

  • Utilizes the nursing process to assess patient status, collaborate with attending staff to intervene appropriately, and evaluate patient response.
  • Participates as a member of the interdisciplinary team addressing patient and family needs.
  • Observes, evaluates, and contributes information regarding psychosocial, spiritual, physical, and financial conditions affecting the patient and family.
  • Serves as an advocate for the patient/family unit.
  • Develops and maintains contact with appropriate community agencies and services to promote inter-agency cooperation and facilitate referrals.
  • Facilitates and supports decision-making around end-of-life planning, including discussions about treatment options, advance directives, and communication of wishes with family and friends.
  • Maintains patient records in accordance with state, federal, and organizational requirements.
  • Engages in professional development related to position responsibilities.
  • Participates in palliative care committees and task forces.
  • Performs other duties as assigned.
