About The Position

As a Nurse Practitioner (NP) in our Palliative Care Program, you will provide expert care to patients facing acute and chronic conditions, working closely with physicians and interdisciplinary care teams. You will be instrumental in managing end-of-life care, symptom management, and fostering comfort and dignity for patients and their families. This role offers a unique opportunity to work in a compassionate, patient-focused environment, where your expertise in palliative care will have a direct impact on improving patients' quality of life. You will report to the Director of Nursing Support Services and collaborate with a dedicated team of healthcare professionals. The ideal candidate will bring strong clinical skills, an ability to assess and manage complex health conditions, and a deep sense of empathy to address the physical, emotional, and psychological needs of patients.

Requirements

  • Active California RN License and Certified Nurse Practitioner status.
  • Strong experience in palliative care, hospice, or related fields is preferred.
  • Excellent skills in assessment, diagnosis, and treatment.
  • Strong interpersonal communication skills, with a compassionate and empathetic approach to patient care.
  • Highly organized, self-directed, and motivated with the ability to work independently and as part of a collaborative team.

Nice To Haves

  • Education and/or work experience in palliative care or Hospice.
  • Proficiency in developing care plans, managing chronic conditions, and providing end-of-life care.
  • Comfortable prescribing medications and treatments as permitted by licensure.
  • Experience in coordinating care teams and working with interdisciplinary healthcare providers.

Responsibilities

  • Manage Day-to-Day Operations: Oversee the day-to-day operations of the Palliative Care Program, ensuring quality care is delivered to all patients.
  • Coordinate Patient Services: Lead the coordination of direct and indirect patient services, working with a multidisciplinary team to ensure seamless care delivery.
  • Staff Orientation & Development: Assist in the orientation of new personnel and promote ongoing development of clinical staff to ensure excellence in patient care.
  • Palliative Care Delivery: Provide comprehensive palliative care through health assessments, diagnosing conditions, developing care plans, prescribing medications (within licensure scope), and coordinating care with the patient’s family and other healthcare providers.
  • Collaborative Care Planning: Work with physicians and interdisciplinary team members to create individualized care plans tailored to the patient's needs.
  • Symptom & Pain Management: Use a patient-centered approach to discuss end-of-life issues, symptom management, and provide comfort to patients and families during difficult times.
  • Referral and Consultation: As needed, make referrals or consult with other specialists to ensure holistic, multidisciplinary care is provided.
  • Community-Based Care: Utilize community resources to design and direct care that meets the needs of patients and their families in their home environment.
